The South Carolina Department of Labor, Licensing and Regulation also determined that the home shopping network’s safety program wat the 560-employee facility was deficient, according to story.

The determination and fine stem from the death of Agnes Ann McKithen Carter, 45, who was a QVC warehouse assistant at a distribution center in Florence, S.C.

She was 30 feet in the air on a forklift grabbing merchandise when a newly trained forklift operator drove into Carter’s aisle and hit her forklift. Although Carter was in a harness, she fell down with her forklift and died at a local hospital.

QVC has until May 4 to address safety concerns and to pay its fine, the story said. In a statement, QVC expressed sadness over Carter’s death and said it was dealing with safety issues.

The network also said that the employee who hit Carter’s forklift is no longer with the company.

Carter, who had a lovely smile, is survived by her husband, who she recently married, and four kids.

Her mother told the local newspaper that it is pursuing legal action against QVC.
